Step 1: Concern Voiced
*A concern is voiced regarding the progress of a student to the special
education teacher.
*A referral may come from a screening test, or come from the student, the
parent, a
classroom teacher, or from other district or private agency personnel.
*A review of the cumulative record, an interview with the referring person, and
possibly a
classroom observation are minimum requirements for the pre-referral stage.
Step 2: Pre-referral Interventions
*Alternative interventions are attempted and effectiveness documented.
Step 3: Documentation of effectiveness
*Effectiveness of interventions is documented.
*If interventions prove ineffective, the evaluation may begin.
If interventions are not effective:
Step 4: Referral Forms Completed
*The special education teacher contacts the parent via phone or in person,
indicating
that the school district will be seeking their permission to conduct an
evaluation.
*The special education teacher mails the parent the prior notice/permission to
evaluate form, the parental rights booklet, a childhood history form, and
a transition
survey (if child is 15 or older).
Step 5: Evaluation Initiated
*The child is evaluated based upon the area of suspected disability.
Step 6 Evaluation Team Meeting Held
*All evaluators meet to discuss if the child may be eligible for special
education.
Step 7: IEP Meeting Held
*The district shall mail to the parents a completed prior notice form
informing them of
the placement date, time, place, and persons invited to the meeting.
*IEP team meet and discuss evaluation results, eligibility, and the IEP (if
the child is
eligible for services).
